U.S. Department of Transportation Physical Exam

Dr. Ryan Thomas is a Certified Medical Examiner for the U.S. Department of Transportation (DOT) and performs DOT physical exams for drivers.  All commercial drivers whose current medical certificate expires on or after May 21, 2014, at expiration of that certificate must be examined by a medical professional listed on the National Registry of Certified Medical Examiners, and Dr. Thomas is on the registry.  Only medical examiners who have completed the training and successfully passed a test on the Federal Motor Carrier Safety Administration’s (FMCSA) physical qualification standards are listed on the National Registry. 

 

The FMCSA sets the medical requirements for drivers.  To schedule a DOT physical exam with Dr. Thomas, please contact DOT Express Physical Exam or Thomas Chiropractic.  To save time at your appointment, we encourage you to print the Medical Examination Form, complete section 1 of the form, and take the entire form to your appointment with Dr. Thomas.  If you have any questions, please contact our office at (724) 994-8587.

  • Certification typically 30 minutes or less

  • Driver receives original DOT long form and laminated travel card

What you will need for your DOT Exam​

 

1.  Print the DOT Medical Exam Form, complete section 1 and bring the entire form with you.

 

2.  Bring your driver's license and current DOT medical card (if applicable).

 

3.  Bring a list of any current medication you are taking along with any reports of any current medical conditions.
